Active Duty

Blanchfield’s Soldier-Centered Medical Homes are team-based and holistic concept which allows us to provide you more personalized care with multiple ways to access your care team. From office visits and walk-in services to telephone consults (TCON) and secure messaging, you have even more ways to get the care you need.

This model is based on the belief that your well-being should always be at the center of every decision about your health. With this new approach, you will receive more personalized and coordinated care, supported by an entire team to maximize overall wellness. You no longer have just a primary care provider, but an entire primary care team!

Soldiers within the 101st Airborne Division (Air Assault) and Fort Campbell are assigned to a Soldier-Centered Medical Home aligned with Soldier's units. Soldiers receive their care at one of the following medical homes: Byrd Solider-Centered Medical Home, Campbell Army Airfield, LaPointe Soldier-Centered Medical Home, or the Warrior Care Clinic. Soldiers assigned to a garrison or medical unit are assigned to Blanchfield. Soldiers have unique requirements to maintain their medical readiness to support the overall readiness mission. Each unit's primary care manager can assist Soldiers in ensuring their medical readiness requirements are met.
